Add/Remove Note Numbers Dialog

Use this dialog to remove note numbers from a cross-reference manually. Clear check boxes for the notes that should be hidden.

It is also possible to add back note numbers that were previously in the cross-reference but were removed. Check boxes beside the notes to restore to the cross-reference.
